Rhinoplasty, one of the most commonly performed surgeries in the world, can be referred to as the “circumcision of the modern age.” The increasing demand for rhinoplasty has also led to an increase in the supply of services. Today, it is the most frequently performed surgery by ear, nose, and throat, as well as plastic surgeons.
Rhinoplasty, although perceived as a simple procedure, is a serious and significant medical intervention with important consequences. Like other vital and functional surgeries, it involves processes such as informing, prepping, testing, treating, monitoring, and care. It serves a therapeutic purpose like all medical surgeries but also carries certain risks. For example, the healing time is generally short but can vary from person to person. While patient satisfaction is generally achieved, there is no absolute guarantee of perfect results. A social media survey found that rhinoplasty satisfaction was reported at 83.6%. Interestingly, the satisfaction rate was significantly higher in women compared to men. It was also noted that women were more comfortable expressing their dissatisfaction than men. Dissatisfaction increases when revision surgery is required. However, dissatisfaction can arise independently of complications or the need for revision. This is because satisfaction measurement is generally based on subjective criteria. Revision rates in rhinoplasty range from 4.4% to 9.8%. However, our personal opinion is that revision rates may be higher.
Another important issue is whether the patient coming with the desire for rhinoplasty is suitable for this surgery. As physicians, we have an obligation to evaluate the patient in a biopsychosocial context. First and foremost, the sources of the patient’s motivation for the surgery are crucial. Patients who come for external reasons such as the attitude of their close circle or the influence of social media, with unrealistic expectations, or those who have recently experienced problems in their social relationships (divorce, separation, job changes, etc.) should be approached with caution. Patients’ sensitivities should be well evaluated, they should be informed in detail, including the risks and complications, and they should be provided with the opportunity to decide without being under any influence. The physician should guide the patient with the information provided.
In rhinoplasty, it is essential to balance the aesthetic appearance of the nose with its functionality. Our effort in surgeries should be better than before. Pursuing perfection can lead to mistakes for both the patient and the doctor. However, this does not mean that we will not do our best. Besides knowing what to do during surgery, it is also crucial to know where to stop. Sometimes, the pursuit of perfection becomes the enemy of good.
The patient should know that nose surgery can improve the shape of the nose and make breathing more comfortable. However, the success of the surgery does not directly affect personal and romantic relationships, for example. The post-operative confidence of the patient plays an important role in such cases. On the other hand, an unsatisfactory surgery is not the end of the world. In such a situation, seeking a solution with a clear mind is possible. However, it is recommended not to poison your life and that of your loved ones.
In this article, we tried to address the less-talked-about aspects of nose surgeries. Of course, everything is for the well-being of our patients. But being realistic is also necessary to take precautions and avoid unnecessary sorrow.
